"1st International Congress on “Prediabetes” and the Metabolic Syndrome – A Review"

Summary :
In this presentation Prof. Davis gives an overview of salient conclusions presented at the 1st International Congress on “Prediabetes” and the Metabolic Syndrome held in Berlin, Germany from April 13th-16th, 2005.

The term “prediabetes” has previously been used in a retrospective sense to describe the period preceding the diagnosis of diabetes, but now has come to mean IFG or IGT or both. Prof. Davis discusses the prevalence of IFG and IGT, and the increased risk of macrovascular disease associated with prediabetes beginning within the euglycaemic range.

The new IDF consensus on the metabolic syndrome was another major topic of interest. The IDF consensus meeting of 2004 aimed to come up with a practical definition of the metabolic syndrome, and the resulting consensus definition incorporates ethnic-specific cutoff values for waist circumference as a measure of central adiposity, and some other new features and considerations.

Different studies have investigated the prevention of diabetes in prediabetic subjects with lifestyle modification and/or pharmacological agents. Important conclusions from the U.S. DPP and Indian Diabetes Prevention Programme (IDPP), Finnish DPS and STOP-NIDDM trials are discussed, as well as the HHS, VAHIT and BIP studies which looked at the effects of fibrates in patients with the metabolic syndrome.

Prof. Davis gives an overview of ongoing intervention studies in subjects with prediabetes or diabetes plus at least one other CVD risk factor, and a recap of presentations on the health economic aspects of treating the metabolic syndrome. Finally, recent studies point to the importance of acute hyperglycaemia in predicting short- and long-term outcomes after acute myocardial infarction, and hence a need for strict glycaemic control following the event.
