Diabetes is a serious and rapidly growing health problem in the United States and the world, with Native Americans and other indigenous groups bearing a disparate burden of diabetes and its complications. Inequalities in access to health care as well as health care quality worsen the impact of these disparities. Moreover, mistaken beliefs and fears about diabetes—that it cannot be prevented and its complications are inevitable—are widespread among indigenous peoples and further compound this public health crisis in indigenous communities. However, recent scientific studies, demonstrating that it is possible to prevent or delay type 2diabetes and that good diabetes control can reduce complications, bring hope to indigenous communities that the epidemic of diabetes can be reversed. The Indian Health Service (IHS), an agency within the United States Department of Health and Human Services, was established in 1955 as the principal federal health care provider and health advocate for American Indians and Alaska Natives. The agency is a comprehensive, primary health care system of hospitals and clinics located on or near Indian reservations in 35 states. The National Commission on Diabetes identified the growing problem of diabetes in American Indian and Alaska Native communities in the 1970’s and recommended that Congress establish a special program to address diabetes within the Indian Health Service—the IHS National Diabetes Program. Now the most comprehensive and rural system of care for diabetes in the U.S., the program combines both clinical and public health approaches to the problem of diabetes. Surveillance and evaluation of program activities over time have shown many successes in the care and treatment of diabetes in American Indian and Alaska Native communities.
