Summary :
In this presentation Prof. O'Brien talks about the effects of major weight loss in type 2 diabetes.
A direct relationship exists between BMI and the risk of developing type 2 diabetes (1,2). Obesity promotes insulin resistance, hyperinsulinaemia and eventual beta-cell failure, so can weight loss “cure” diabetes before beta-cell failure has occurred?
For severely obese individuals in whom conventional weight loss programs are not expected to achieve sufficient weight reduction, the Lap-Band surgical procedure presents a safe and effective option. Prof. O'Brien talks about his experience having treated over 1800 patients with the procedure.
In severely obese type 2 diabetic patients, Lap-Band surgery has been associated with significant metabolic improvements and even remission of diabetes in some patients (3). Prof. O'Brien describes the beneficial changes in insulin sensitivity and beta-cell function seen one year after the operation (4), and introduces an ongoing randomized controlled trial in type 2 diabetic patients, comparing the effects of optimal care with or without Lap-Banding.
