"Polymorphism -308 G/A of the TNF-(alpha) Gene: Effect on plasma soluble TNF-receptor 2 (sTNFR2) according to Degree of Glucose Tolerance (IGT, DM2), Insulin Resistance (IR), and other Related Parameters. A Population Based Study (The Segovia Study, Spain)"

Summary :
To investigate the relationships between the -308 G/A TNF-(alpha) polymorphism on plasma sTNFR2 levels, IR and other related parameters 436 unrelated subjects (49.1% males; 35-74 years) from a cross-sectional population-based survey in the province of Segovia (Spain) were studied.

Measurements: BMI, waist circumference (WC), sagittal abdominal diameter (SAD), systolic and diastolic blood pressure, lipid profile, fasting and 2 hrs glucose (ADA 1997). Insulin, proinsulin, leptin, adiponectin (RIA), PAI-1, t-PA and sTNFR2 levels (ELISA). IR was estimated by HOMA. -308 G/A TNF-(alpha) genotypes were determined by PCR-RFLP.

Genotype distribution GG: 72.9%, GA: 24.3%, AA: 2.8%. Glucose Tolerance: NG: 75.2%, IGT: 16%, DM2: 8.8%. The A allele was associated with higher prevalence of hyperglycaemia in the overall population. Among diabetic subjects (n=59) sTNFR2 levels were positively associated with BMI, WC, SAD, fasting proinsulin and t-PA, and negatively with HDL-cholesterol. Moreover diabetic subjects carrying the -308 A allele had significantly higher sTNFR2 levels than non-carriers.

Our results indicate that the -308 A allele of TNF-(alpha) is associated with hyperglycaemia independently of other risk factors such as obesity and IR. Among diabetic subjects sTNFR2 levels are correlated with parameters of the Metabolic Syndrome such as obesity and HDL-cholesterol. Moreover the -308 A allele may predispose to higher sTNFR2 levels among diabetic subjects.
