"Primary Prevention of Type 2 Diabetes - A Matter of Maintaining the New Lifestyle"

Summary :
Object: To study short- and long-term effects of an intensive lifestyle change programme.

Method: Subjects with IGT and obesity (BMI>27) were randomised to two different intervention programmes based on low-fat, high-fibre diet and regular physical exercise. The intensive group (n=100) participated in a one month learning and training session using behavioural modification techniques and conducted in a full-board wellness centre. The usual care group (n=94) was randomised to a one-hour counselling session. Follow-up was done after 1, 3 and 5 years.

Results: At 1-year follow-up, in the intensive versus the usual care group there was a 70 % reduction in worsening to type 2 diabetes (T2DM). Body mass index, waist circumference, blood pressure, fasting proinsulin, leptin, PAI-1 activity, tPA-antigen and LDL-cholesterol were all significantly lowered in the intensive compared to the usual care group. Furthermore, healthy eating habits had improved and physical activity along with HDL-cholesterol had increased. At 5-year follow-up, most of the attained differences between the groups in risk factors as well as in diabetes incidence had disappeared. This was mainly due to adherence problems. However, fasting proinsulin was still lower and HDL-cholesterol higher in the intensive group.

Conclusions: Lifestyle change induces a multitude of beneficial metabolic effects that may protect against cardiovascular disease and T2DM. However, these metabolic effects only persist as long as the lifestyle regimen is maintained.
