Summary :
The challenge to effectively manage type 2 diabetes has perhaps never been so critical. We are now aware of the increasing number of people who will develop diabetes over the coming years. There is also the recognition that diabetes is a complex disorder with major disturbances in glucose control and blood pressure and lipid abnormalities leading to not only the microvascular complications of diabetes but also perhaps the most important vascular complication of diabetes, coronary artery disease (CAD).
We now have an increased understanding of the pathophysiology of type 2 diabetes. The combined defects of abnormal insulin secretion and increased insulin resistance lead to a series of abnormalities that promote vascular complications including CAD. The UKPDS data emphasized that the traditional therapies that we have used for decades are not sufficient to treat this disease. Well-known drugs fail within three years to maintain adequate control. The introduction of the TZD drugs has allowed the concept of combination therapies to be carefully considered as part of the essential management program for the type 2 diabetic patient. The future approach to management of type 2 diabetes is likely to include early and aggressive management with combination drug therapies aiming at ever-decreasing glucose targets. The impact of these new concepts of therapy promise to be exciting and meaningful and will require a major change in therapeutic approach on the part of the health care professional treating the person with type 2 diabetes.
Learning objectives :
The participant will review how traditional type 2 diabetes therapy has failed, and learn about promising new concepts in therapy.
