Summary :
In the natural history of type 2 diabetes, the development of insulin resistance, impaired glucose tolerance and finally type 2 diabetes occurs over several years, with no apparent symptoms. This period is significant as insulin resistance is associated with cardiovascular abnormalities. Compensatory hyperinsulinemia maintains blood glucose levels until the point of beta cell exhaustion, when hyperglycemia becomes evident. As seen in the UKPDS, beta cell function continues to decline over time.
