"What Should Governments do to Prevent Diabetes/CVD?"

Summary :
The extent and magnitude of the present and future burden of diabetes and related cardiovascular diseases is now well recognized by the medical community. While effective treatments exist, their use only benefits those who have developed these diseases. The fundamental problem relates to prevention of these diseases. For type 2 diabetes, there is unequivocal evidence that the most important risk factors are obesity and physical inactivity. These risk factors have and continue to increase because of behavioral changes in the population over the past half century. Thus, obesity and its consequences have emerged as the major public health problem of the 21st century.

Addressing issues of public health in the context of disease prevention inevitably involve government policy and intervention. To prevent further worsening of the epidemic of obesity, and hopefully reverse it, sound public health policies need to be instituted at local, national and international levels. The potential range of actions by governments to address this problem is broad and varied. They range from policies that will encourage and promote public education about the problem to actions to change the physical activity and eating behaviors of the public so as to promote obesity prevention of obesity. Such actions may range from redesign of cities and transportation systems to encourage safe walking and other forms of physical activity to changing school curricula to ensure that children exercise regularly and promote better eating habits and behaiours. Potentially, this might be achieved by public education about desirable nutrition, actions to regulate the food industry to improve food labeling, limit advertising, or by subsidizing desirable foods and taxing those that are harmful, e.g. those with a high fat content.

The public health challenge in some ways is analogous to, but more challenging than smoking prevention. Over a 40 year period, at least in developed countries, a marked reduction in the frequency of smoking has occurred mainly as a result of governmental policies and actions. Actions at the governmental level to prevent obesity and the consequent epidemic of diabetes and cardiovascular disease will require even more intense, concerted efforts over many years. The moment has arrived where local, national and international governments need to become engaged, and identify and implement policies and actions, which in the long run, will reduce the burden of obesity and thereby prevent its consequences.
